From a certified medical examiner; the Honolulu office takes it by fax or email.
Any TPR-registered provider. ELDT365 files your completion the moment you pass.
General Knowledge plus any class or endorsement tests you need.
Federal minimum before you can book the practical test.
With a TPR-registered BTW provider; the office verifies it in the TPR before scheduling.

Hawaii county driver licensing offices under the Hawaii Department of Transportation (on Oahu, the Honolulu CDL Office in Pearl City).
Age: 18 for the CDL instruction permit; the counties and HDOT list 21 for the CDL itself (state law sets 19 as the floor for an intrastate-only CDL). 21 for interstate or hazmat..
Hold your permit: 14 days before the skills test.
Permit valid: One year per the HDOT guide; see your county CDL office.
County CDL offices give the three-part test (pre-trip, skills, road) themselves; appointments are made in person at the office.
Hazmat knowledge test at the county office; TSA security threat assessment (universalenroll.dhs.gov) before the H is issued. The H is good for up to five years from TSA approval and the CDL expires on the same date.
Federal rule 49 CFR 380.703(i) says that state qualification requirements for theory instruction do not apply to providers offering that instruction only online. So your state accepts theory from any provider listed on the FMCSA Training Provider Registry, including ELDT365, and no state can require the online theory portion to come from a state-licensed school. States can add their own rules for the behind-the-wheel part, and several do; those are listed below.

What Hawaii adds for the school part: No state licensing layer for theory. Any TPR-registered provider is accepted; the county must see your completion in the TPR before it schedules the road test.

As of February 2022, the FMCSA requires all first-time CDL applicants to complete Entry-Level Driver Training (ELDT) from a registered training provider before taking their CDL skills test.
This applies to all Hawaii drivers applying for a Class A CDL, Class B CDL, or any endorsement including HAZMAT, Passenger, and School Bus.
ELDT365 is registered with the FMCSA Training Provider Registry (TPR) and fully accepted by the Hawaii DMV.
